Lung Transplantation: Surgical Advances, Organ Preservation, and Outcomes

This chapter reviews the current state of lung transplantation, emphasizing innovations in donor lung assessment, ex vivo lung perfusion (EVLP) as a platform for organ reconditioning and evaluation, and advances in surgical technique for single, bilateral, and lobar transplantation. Contemporary immunosuppression protocols, the management of primary graft dysfunction, and the pathophysiology and treatment of chronic lung allograft dysfunction (CLAD) are critically examined. The chapter also addresses the expanding use of marginal donor organs, the role of donation after circulatory death (DCD), and strategies for optimizing long-term post-transplant survival in an era of growing waitlist demand.
